CARE COORDINATOR

We are searching for a dynamic individual to become the CARE COORDINATOR for the unit. This position provides care coordination for the patients in the program.

All applicants must undergo pre-employment drug screening,

which includes testing for marijuana, as well as background checks.

ESSENTIAL DUTIES AND RESPONSIBILITIES: include the following. Other duties may be assigned.

Responsible for the development of care planning and patient needs assessments.

Facilitates life skills groups by the CTP policies and procedures.

Acts as liaison, establishing networks and collaboration with hospitals, community health centers, and state and human service agencies about patient services and needs.

Document admissions, care management needs, discharge, and discharge planning.

Administer case management assessments to identify patient needs and ensure the Addiction Severity Index (ASI) is completed at intake.

Organize a schedule of patient bed dates, transitioning levels of care, and consistent communication with the Medicaid Placement Coordinators.

Assists treatment team with patient intake, transfer, and discharge assessments.

Collaborating with the facility treatment team (medical, psychiatric, and SUD) to identify patients needs.

From available resources, assist patients in meeting their basic needs (i.e., clothing, medical, judicial, religious, etc.).

QUALIFICATIONS:

Education:

An associate or bachelors degree in a behavioral health-related field is preferred.

Obtain the Certificate of Addiction Technician (CAT) certification within 18 months of your hire date.

Obtain case management certification within 90 days of hire.

Experience:

Minimum of two years of experience working in substance misuse and mental health.

Other:

Must have a good understanding of substance use, mental health, and treatment delivery systems.

Advanced computer skills are required.

CPR and First-Aid certification (the agency pays for initial training and renewals).

Must be able to take vital signs, recognize possible medical problems, and refer to the medical staff when necessary.

Valid drivers license and good driving record (must provide a current unofficial DMV driving record upon hire).

Able to communicate effectively in a multidimensional treatment program, including medical, psychiatric, and substance use related.
